Approved Volunteers for Associations

ACT 33 - Criminal Background Check


1. Go to PA’s PATCH website and click on Submit a New Record Check. You will need a credit card to process your clearance.
2. Print your Certificate and make a copy (you’ll need to send one to Harrisburg with your PA Child Abuse Clearance form). Keep one copy for your records.
3. Write down and save your Approval Code Number.

ACT 34 - PA Child Abuse History


1. Complete the ACT 33 process above before you start the ACT 34 process.
2. Complete the PA Child Abuse History Clearance form (download PDF on the right side of this page).
3. Mail with a copy of the PA Criminal Records Check and a $10 Money Order to:

Submit to Parks & Rec

1. Once your PA Child Abuse Clearance has been mailed back to you, take copies of both clearances to the PT Rec Center and deliver to Michele Harmel. Keep copies for your records.
2. Your name will be added to the PT Approved Coaches List.
3. Your clearances will be good for 3 years.
